Patient Portal

Our patient portal allows existing patients to schedule an appointment, request prescription refills and get answers to billing and health questions. To register, ask our front desk at your next visit; you will be asked a few basic questions and for your e-mail address. Established patients can enter the patient portal here.

Before You Visit

  • Bring your insurance card, a photo ID and a list of current medications.
  • For a surgical consultation or second opinion, bring relevant X-rays, MRIs, CT scans and reports.
  • Wear or bring the shoes you use most often, especially if you are being evaluated for pain or orthotics.
